Health Services at Vision Academy Charter School
At Vision Academy, we are dedicated to promoting every student’s physical health and well-being. We offer free health services to families for exams conducted by or at the school. If you have any questions, please contact our school nurse at nurse@vacharter.org.
Annual Health Services
- Vision Screening: The school nurse conducts Vision screening yearly for all students.
- Hearing Screening: Provided for students in kindergarten and grades 1, 2, 3, and 7.
- Height, Weight & BMI: Measured once per year for every student.
- Health Records: A school health record is created upon enrollment and maintained throughout the student’s academic journey.
Required Medical & Dental Exams
Physical Examinations
- Required for all students entering school (Grade1), and again in Grades 6.
- Families are encouraged to use their own physician for these exams (at their own expense), as family doctors are familiar with the child’s medical history.
Dental Screenings
- Required for students in Kindergarten, Grades 1, 3, and 7.
- School Dental Form (revised Feb. 2024) (PDF). This form may be used by both the mandated and dental hygiene programs as the dental portion of a student’s comprehensive health record.
- Parents are encouraged to use their family dentist (at their own expense) for these screenings.
Immunization Requirements
All students must be up to date with the required immunizations to attend school. Parents must submit updated immunization records to the school nurse whenever new vaccines are administered.
Required Immunizations:
- Diphtheria/Tetanus: 4 doses (1 after the 4th birthday)
- Polio: 3 doses
- Measles: 2 doses (on or after 1st birthday)
- Mumps: 2 doses (on or after 1st birthday)
- Rubella: 1 dose (on or after 1st birthday)
- Hepatitis B: 3 doses
- Varicella (Chickenpox): 2 doses (on or after 1st birthday)
- Meningococcal (MCV4): 1 dose (on or after 11th birthday)
- Tetanus, Diphtheria, Pertussis (Tdap): 1 dose (on or after 10th birthday)
Medication at School
Whenever possible, medications should be administered at home. If a student must take medication during the school day, the following conditions must be met:
- Medication must be in its original labeled container.
- Prescription medications must include physician instructions.
- Over-the-counter medications must include clear instructions from a parent or guardian.
- Written parental/guardian permission and a signed liability waiver must be submitted.
